Required Camp Forms

After you register for a camp, you will receive an email with all of the forms you'll need to fill out before your girl joins us for camp. Girls will not be able to stay at camp without turning in the required forms by the due dates listed below.

  1. Record of Health Exam Form and Immunization Record

    Due: June 1, 2020

    Your camper will need to turn in a completed Health Exam Form (PDF), signed by your doctor, and should include an immunization record. Once completed, you'll upload this through your CampDoc portal.

  2. CampDoc Health Profile

    Due: June 1, 2020

    Next, you'll need to fill out your camper's CampDoc Health Profile.

  3. Additional Program-Specific Waivers

    Due: at check-in

    Depending on your girl's program, there may be some additional program-specific waivers for your to fill out. If your camper's program requires additional waivers, they will be emailed to you at your time of registration with instructions on how to complete them.

After you register your girl for camp, you will receive an email from Girl Scouts of Northern California with instructions on how to set up your CampDoc account.

Once you register for CampDoc, you'll receive a “Welcome Invite E-mail” from with information about how to complete your camper’s health information. These emails are usually sent within 1–2 weeks of completing your camp registration. Please set as a safe sender by putting the address in your contacts to avoid accidental delivery to junk and spam folders. Still looking for the welcome invite? 1. Create Your Account Click the link in your welcome email to create a password for your CampDoc account.

2. Complete Your Camper's Health Profile Follow the instructions, and complete the health profile for your camper. If you used the CampDoc system last year, you will still need to sign in and review that everything is reviewed and current.

3. Upload Your Required Documents Upload any required documents to your CampDoc account, such as the Health Exam Form (PDF) that is signed by your medical provider must be uploaded. If you have immunization records, the easiest way to complete that section is to upload them.

Don't have a scanner? No problem! Use a smartphone or camera to take a photo and upload the .jpg just like you would to any website.

4. Make Updates Anytime Return to CampDoc at any time to make updates to your camper’s health information before camp begins.

All campers must have a completed CampDoc health profile by June 1, 2019, or they will not be allowed to attend camp.
